TRANSITION OF CARE (TOC)

What is Transition of Care?

​

TOC is a short term no-cost outpatient care service available to Kern Health Systems members who have recently been treated and discharged from the hospital

SERVICES

• TOC team is available for 30 days after a hospital visit providing unlimited number of visits to help members reach their health goals
• TOC will work alongside your primary care doctor to provide additional services - If a member does not have a PCP, the TOC team can assist with finding a PCP

WHO IS ELIGIBLE?
 

You may be eligible for TOC  if you meet any of the items below:

  • Patients that have recently been admitted or treated in the emergency room
